By completing the form below, YOU AGREE that you are:

• 18 years of age or older 
• Currently living in or returning from a correctional facility to live in the Guilford County, NC area. 
• You have demonstrated a willingness to change your life for the better.
• You commit to this six (6) month non-residential program providing transitional assistance in the areas of counseling & spirituality, professional development training and reentry support services.
